Acknowledgment of receipt of the NPP



Opportunity to object to being listed in the patient director
y and/or having
religious denomination being shared with clergy (“Opt Out” of patient
directory/clergy)



Opportunity to object to disclosures of protected health information (PHI)
to family and friends or limit disclosures to specific persons (“Opt Out” of
communications with Family and Friends)

Documentation for Receipt of NPP and “Opt Outs” on General Consent
for Treatment Form:



The completed General Consent for Treatment Form will be maintained in
the patient’s medical record.



Registration staff will ente
r the acknowledgment of receipt of the NPP on
the SMS registration screens.



If a patient selects any of the “Opt Outs” the Registration staff will enter
the selection on the SMS registration screens.

Tracking and Notification:
If an “Opt Out” is selected
a flag (see
Flags
below) will be set for the patient on the following reports and screens:



Census Online and Paper will be flagged with a “X” to note that an “Opt
Out” has been selected



Information Desk printed reports and system will display one of the “O
pt
Out” Flags listed below



Appointment Schedule reports for outpatients will display one of the “Opt
Out” Flags listed below



Removed from Community Clergy report if “Opt Out” of clergy list (not
UNC HCS Pastoral Care)



Any inquiry on patient in SMS system w
ill show “Opt Outs”



Any inquiry on patient in Web CIS will show “Opt Outs” and
acknowledgment of receipt of NPP
