1


Introduction









This chapter gives a brief overview of following

Ration Card Services:


1.

Alteration of Ration Card.

2.

General Query for Alteration of Ration Card.

3.

Issuing Ration Card Certificate.



It describes the functionality of the processes in brief.













What is Alteration of Ration Card


A Ration card is al
tered when



The family is moving to a new address in a different ward.



A new member is added to the existing family.



Name of family member has to be removed; a surrender certificate has to be submitted in
this case.



Name of one or family member is changed.



The name of ration shop has to be changed in a ward where other shops are also existing
for reasons such as easy access etc.



Overview For Status Inquiry


For Checking Status of Alteration of ration card, an applicant can apply at the
CHO
i
CE center o
r RMC Center.




Application status can be viewed at a center by Entering Application number or the
name or the ration card number, the result will be showing the current status of the
application.



Overview For Alteration

For Alteration of ration card, an

applicant can apply at the choice center or RMC
Center.




CHOiCE Agent/RMC Operator will enter the data as specified in application
along with scanned copy of Ration Card & Certificate From the authorized ration shop in
his ward.



As the application is ente
red in the system, then the corresponding clerk
(Depending upon type of ration card) can view the same.



Clerk can search the application by application no. Or name or ration card
number. etc.



If Ration Card is of the APL category then only APL Clerk can vi
ew the
application for the further processes (i.e. approval or rejection).



If Ration Card is of any other category, then BPL Clerk can view the application
entered.



Clerk can approve or reject the application entered but he has to enter the reason
for disa
pproval.



If Clerk approves application then it moves to second step of approval i.e.
OICRTN.



OICRTN has three options APPROVAL, REJECTION, UNDER
CONSIDERATION.



In case if officer rejects or put the application under reconsideration then he has
to specify t
he reason for the same.



If the application is approved then a ration card certificate is issued at RMC that
has to be collected in person & the original ration card has to be submitted.

Menus/Comma

This chapter lists down the purpose of all the menu and sub menu items for Ration Card
Services for Choice Center user.




The LogIn Screen


The user has to click the Continue button in the USERNAME/PASSWORD
AUTHENTICATION and BIOMETRIC AUTHENTI
CATION screen to enter in the
common menu screen
.
The user has to give his/her User Name, Password in the
screen and Thumb Impression in the biometric device to authenticate him/her and log
in into the common menu screen for CHOiCE.

In this screen, the user can select any of the services under CHOiCE, by clicking on
the name of the services under Ration Card like Application for Alteration of
Ration card.

This Process will be used when an applicant
comes to submit an application for Alteration
of Ration card.




After clicking the application menu item for
Alteration of Ration card, the entry

screen for
application data will be displayed to the entry
operator.

General Query for Alteration of
Ration Card



After clicking the General Query for
Alteration of Ration Card menu item a screen
will appear where the user can see the current
status of a
ny submitted application. The user
can also enter some searching criteria (like
application reference number, from date, to
date, head of the family name,
father/husband’s name) to locate the required
application.

Instructions for using the screens


This chapter deals with the step wise instructions for using all the screens of the
services under Ration card for CHOiCE Centre user.















































Instructions

For

Application for Alteration

of

Ration Card

APPLICATION AT RMC OR CHOICE AGENT LEVEL




After clicking the ‘Application for Alteration of Ration Card’ menu under ‘Ration Card’, a screen
containing general i
nstructions along with information about Application, Status, eligibility and
Enclosures will appear. This screen also contains two buttons named ‘Application form’ and
‘Back’.



The user has to click on the Application form button for entering into the appl
ication form screen.
The user can see the keyboard layout for hindi language by pressing the ‘Keyboard Layout’ button
which will also appear at the top right side of the application entry screen.



All the fields marked with red coloured ‘*’ are mandatory f
ields.



Firstly user has to fill the required fields to make search for the ration card available. If user
doesn’t fill any field & presses search button then all the records will be displayed.



As user click on one of the Ration Card Number (having blue co
lor).



Next form is opened which is displaying



Name of Municipal Corporation



Application Date Field displays today’s date.



Ration Card Number



Type of Ration Card

sub heading is GENERAL INFORMATION

In this section, user can select the type of Alteration

required from the following options

1.

Addition of new member

2.

Alteration in existing family member name

3.

Change in address

4.

Change in Ration Shop

5.

Alteration in name of head of family.




User has to select one of the above options. By doing so, the section dea
ling with the selected
alteration will be enabled and the user can make the desired changes.

If the user chooses option:

1.

The user has to enter the details of the new member like name, age, relation with head of
family and voter id number if applicable

2.

The

changed name has to be entered for the family member.

3.

New Address has to be entered by the user.

4.

A new Ration shop has to be selected from the right frame of the window after pressing the
selection button.

5.

The changed name of the family head has to be ent
ered.






After filling this application. Next page asks user in read only mode & asks whether he is sure
that application is filled correctly.



If user presses Agreed button then a form opens, which ask the path of necessary documents,
req
uired (i.e. scanned copies).



Then after a form opens which is for the validation with smart card.


APPLICATION AT CLERK LEVEL




When APL or BPL Clerk enter using their login. They can view all the application entered. APL
can view only those applications w
here ration card type is APL. Others would be visible only to BPL
Clerk only.



Or they can search for the record by passing application no. or the name or date etc.



Now if clerk presses on search button without entering any information then it displays all

the
records available.



Now clerk select one of the record by clicking on the application number (blue color on the left
side of the records result).



Clerk is having two options APPROVAL & REJECTION.



If he rejects then he has to mention the reason for the
same.



If he approves then the same record can be viewed by OINRTN.


APPLICATION AT OFFICER INCHARGE LEVEL




Officer can view applications approved by clerk.



The application reflects the changed fields, which is recognized by a new label in front of these
fi
elds.



Here Officer is having three options APPROVE, REJECT, UNDER CONSIDERATION.



If officer approves then altered the clerk at RMC, which has to be collected in person & return the
original ration card, can issue certificate.



If the officer rejects or pu
t the application under reconsideration then he ahs to give the reason for
the same.

General Query For Surrender of

ration card


After clicking the general query for new ration card menu item under ration car
d, a screen will
appear where the user can view the status of any application or more than one applications. The
user can view the status of all the applications entered by clicking the search button or for viewing
the status selectively, he/she can give d
ata in the searching criteria (like application reference
number, application date (from), application date (to), head of the family name and
father/husband’s name) and click on the search button and based on those searching criteria the
current status of
the application along with other information will be displayed like below:
