Discovery Project Phase I
Infographic on Epidemiology of Disease States
The purpose of the HAP Discovery Project is for you to become intimately
acquainted with a
disease state in which you are very interested about. The HAP Discovery Project will last over
the rest of the school term.
Cloud Based Tools
For Phase I of this project, you will be :
1.) researching the epidemiology, etc. of the disease state by collecting, and ana
lyzing data about
the disease state you choose at the world wide level, national level, and community level.
must provide MLA citations for all resources and must acquire raw data and then create images
of the data.
2.) developing a few survey questio
ns to collect information at the community level.
We will do
this collaboratively using Survey Monkey.

Step 1: Let’s brainstorm our disease states as well as the type of questions and statements you
want your infographic to communicate.
Questions for survey should be brainstormed.
Step 2: Write a 2 page paper with MLA citations (endnotes) with basic information about the
disease as well as the key data that you wish to use in your infographic.

about the disease, prevalence of the disease in the world vs. US vs. community, Statistics re:
prevalence in difference ethnicity, socioeconomic lasses, etc.
First collect information about the

your infographic. REMEMBER: Keep the end in mind! What is it you
want to convey?
The infographic is due on Tuesday at the beginning of class.
