Corrective actions required for restart are completed and address the incident causes. This could
include any or all of: repairs, alterations or modifications, requi
red monitoring, temporary equipment,
mitigations.



A review has been conducted to assess implications for similar equipment or barriers on the asset



The Hazards and Effects Register has been reviewed and updated, as necessary, as it applies to this
incident
.



Incident report, if available, and other data and documentation are attached.

Statements of Fitness requirements specified in AI
-
PSM Application Manual Item 7.1 have been
met where applicable:



Process safety risks have been identified and documented and
are either managed to ALARP or
appropriately being managed to ALARP through application of HEMP;



Employees or contractors executing HSSE Critical Activities are competent and fit to work;



HSSE Critical Equipment meets its Technical Integrity requirements;



Design and construction of Asset modifications meet the design and engineering requirements
(including DEM 1, as applicable);



The Process Safety Basic Requirements (i.e. DEM 2) are met, as applicable;



Procedures are in place to operate HSSE Critical Equipm
ent within its Operational Limits.



Modifications, if made, are complete and have been authorized as specified by our DSM
Management
of Change

requirements.

The facility therefore meets the criteria necessary to re

introduce hydrocarbons.

1.

INTENT

that necessary controls are in place to
allow

a safe restart.

The SoF

PSSR

aims to apply a
formal process to aid the
Production Unit Manager, acting on behalf of the
Asset Manager
(the
Site General Manager), in
confirming those controls are indeed in p
lace.

2.

GUIDANCE

These guidance notes clarify

the
requirements for
start
-
up on new or modified assets and
restart
following major shutdowns and process upsets to com
ply
with the S
oF
as specified in the Shell
Group AI
-
PSM Application Manual

and to comply with the
Readiness Review/PSSR
requirements

in the DSM MSRM and the
DSM Management of Change Process
.

Within the scope of this document, the AI
-
PSM
Application Manual
call

oF

when:

tarting or commissioning a new Asset or a Modification to an existing Asset;



R
estarting an Asset after an incident involving uncontrolled shutdown, or an overhaul or a
turnaround, or when the Asset has been subjected to conditions
outside the operational
limits

or when the Asset has
experienced environmental conditions beyond the original design
parameters.

To comply with these requirements, the SoF


PSSR form is to be completed along with
conducting a PSSR using the PSSR checklis
t (refer to BBS
02.05.7235
).

3.

APPLICATION OF SOF
-

PSSR

3.1

NEW ASSET

Startup of a new asset should follow the design and construction of the asset
using the DSM
project processes. Completion of the SoF


PSSR is the final step prior to bringing up the unit.
The project may have a SoF that is completed and signed by the Project Manager and that may
be substituted for this SoF


PSSR.

3.2

MODIFICATION

OF AN EXISTING ASSET

Modification of an existing asset is to follow the DSM Management of Change requirements. This
SoF


PSSR is the Readiness Review that is required as part following the MoC Process.
Depending on the type of change and following the re
quirements of the MoC Process, additional
check sheets may be added for the PSSR checklist provided (refer to BBS
02.05.7235
).

3.3

MAINTENANCE AND T
URNAROUNDS

Statement of Fitness following day
-
to
-
day maintenance is deemed

to be adequately covered by
a properly functioning and effective Permit to Work system, hence no separate SoF needs to be
filled in. (De
-
)isolations, leak testing, etc. is covered b
y the Permit to Work system

and
e
quipment start
-
up is covered by existing operating procedures.

However, for significant maintenance shutdowns (all of the DSM turnarounds are included in this
category) additional assurance in the form of a SoF is required.

A significant maintenance
shutdown
is characterized by:



High spend and/or,



Long duration and/or,



Level of invasiveness (e.g. multiple vessel entry) and / or,



Control system replacements or major upgrades

Uncontrolled Shutdown:

An uncontrolled shutdown is defined as “a shutdown of equipment where the equipment
shuts down
(and blows

down/relieves where intended)
in a sequence,
a timeframe,

or
following a logic that is not as per the design”
.

Conditions outside the
operational limi
ts
:

For the scope of this document and of the SoF document; the term “conditions outside
operational limits” as written in the AI
-
PSM Application Manual, is understood to mean
“conditions outside engineering constraints”. The most stringent engineering con
straint is
often the IPF trip function, or can be the relief valve setting where the design does not
include a trip function.
