Security Administration and state Disability

Determination
Services (DDS) to increase use of

lectronic options for
submitting medical records

or Texas DDS
Consultative Examination reports

related to
disability claims. If Texas DDS needs

additional
information about a patient who has

filed a Social Security
disability claim, you will re

ceive

a request letter for
medical evidence and

other relevant health rec
ords. The
requested

information helps Texas DDS and Social
Security

Administration determine if your patient qualifies

for disability. You can choose the method of send

upload files instead of printing them or writing them
