Instructions for completing this form

This form is to be used only by
individuals in secondary schools

applying for access to the QSA’s secure (restricted)
website. Acces
s to this website is restricted to schools and approved individuals located in schools.

The QSA uses a unique identifier known as the BIC (Brief Identification Code) and password to provide this access. BICs
may be assigned to an individual and/or a school

and various levels of access are then assigned depending upon
requirements.

All Queensland schools have been assigned a QSA School Organisation Identification Number, BIC and password. A list
of the assigned QSA School IDs and BICs can be found at
https://www.qsa.qld.edu.au/qsa_secure/jsp/schoollist.jsp
.

Only individual BICs are assigned by completing this form.

Please note that access to the NAPLAN and QCATS data tool applications is

via
School BIC only

and is not assigned to
individuals. Access to the Assessment Bank is via separate individual registration at the Assessment Bank website.

NB: This form is not to be used to request access to NAPLAN, QCATs or the QSA Assessment Bank.

Upon acceptance and processing of this form, your password will be emailed to the address provided.

Brief Identification Code (BIC) / User ID

Your BIC is an eight
-
character code that you nominate t
o us, and it is used as a personalised identifier. Your BIC helps
us to correctly identify your records in our database. Once you have nominated a BIC, you should always quote it exactly
on all QSA forms that request it.

Your BIC must:



start with four nume
rals and end with four letters (e.g.1710PSMI)



be easy for you to remember



never be changed



be unlikely to match another person’s BIC (e.g. 1234ABCD is not acceptable).

Create your BIC this way.



Make the first four digits the day and month of your birthday
(ddmm)



Make the last four characters your mother’s first initial and first three letters of her surname at birth

(e.g. if you were born on 12 January and your mother was born Pat Walker, then your BIC would be 1201PWAL).

Access level

Access to the QSA restricted website and your school's data will be restricted by the BIC. Your identity will be verified by
password. Principals can authorise access to staff members by completing this form. Princ
ipals can also request access
to be removed by using this form and including the staff member’s details and marking delete against the access level.

School staff can be granted any combination of access types. Access can be modified at any time by completing a new
form. We will ask you to review access at the beginning of each year.
