novel treatments for rare di seases such as primary i mmu
nodeficiency di sorders, pose the need for sci enti sts and
researchers to present thei r studies and experiences i n scientific i nternational congresses for i mproving the human
ki nd heal th and qual i ty of l i fe. In thi s sci enti fi c meeti ng, the mai n theme wi l l be

Prevalence of allergy and
asthma has risen exponentially in the past 3 decades especially among children and
adolescents. Approximately 30
-

40 percent of the world´s population suffers from allergic diseases. This allergy and
asthma epidemic has caused a great burden not only in te
rms of hospitalization, acute care utilization and health care
costs but also of reduced participation in family life, of lost school days and days lost from work.

Public, group or individual education is one of the main principles of allergy and asthma ma
nagement, emphasized in
the global health strategies and medical guidelines. The theme of world Asthma Day (WAD) has been “You Can
Control Your Asthma” since WAD 2007, emphasizing the role of educating the patients for controlling their asthma.
Actually in

some community
-

based surveys up to 41 percent of asthmatic patients under treatment were not well
controlled and had decreased quality of life. Education will increase public awareness about the basic facts, nature,
heredity, causes and triggers of the d
isease, enhance compliance of patients, promote self
-
management plans, improve
correct use of medication, knowledge of the patients and participation of their families for identifying and avoiding
related environmental risk factors and thus optimum control

of symptoms. Following a written action plan to prevent or
handle exacerbations, monitoring symptoms and peak flow values in persistent asthma to adjust medication
accordingly, training to improve stress
-
management and reduce the negative social effects o
f illness
-
specific problems
(in the case of atopic dermatitis) and carrying an epinephrine auto
-
injector and Medic
-
Alert information (in the case of
anaphylaxis) are the other goals of health education in asthma and different allergies. In the case of
Immu
nodeficiencies, promoting public awareness & knowledge of the practicing physicians of primary
Immunodeficiency disorders (PID), educating the patients and household members how to prevent infections and have
genetic counseling or prenatal screening are th
e main objectives.

The results of our previous studies in “Immunology, Asthma and Allergy Research Institute” (IAARI) also have showed
the main risk factors for “Acute Severe Asthma in Iranian Children” to be lack of education for patients and their
famili
es and non
-
compliance regarding the use of inhaled corticosteroids. In this article some of our recent educational
and research experiences in Iran are addressed.

Background:

Primary immunodeficiency diseases (PIDs)

are rare genetic disorders. The most of PIDs are lethal in
childhood due to serious infections. Hematopoietic stem cell transplantation remains the sole curative treatment for
some types of PID.

Materials and Methods:

We prospectively analyzed the outcome

of 43 pediatric patients with Leukocyte Adhesion
Deficiency type one

(n=16), Wiskott Aldrich Syndrome

(n=7), Griscelli Syndrome

(n=6), Familial Erythrophagocytic
Lymphohistiocytosis

(n=4), Severe Combined Immunodeficiency

(n=4), Chediak
-
higashi

(n=3), Hyp
er IgE Syndrome

(n=1), Chronic Granulomatous disease(n=1) and Primary CD4 Deficiency

(n=1) who had underwent HSCT from 2006
to 2012. Twenty
-
nine patients were male. The median age at transplantation was 3.3 years (range: 3months
-
14years).

identical sibling donors (n=21), fully matched other related donors (n=8),
unrelated mismatched cord blood donor (n=8), one locus

mismatched other related donors (n=3), HLA

patients received reduced

methylprednisolone were used as Graft

versus

host disease (GvHD) prophy
laxis regimen.

Results:

The median numbers of MNC and CD34 injected were 6.37×108 cell/kg, 4.44×106 cell/kg, respectively.
Except two, all patients engrafted successfully. At the present time, 34 patients with median follow
-
up of 33 months are
still alive

and 32 of them are disease free without evidence of recurrent infection. The causes of death were sepsis and
GvHD.

Conclusion:

As pre
-
transplant infections in PID patients lead to rise in mortality rate, early transplantation and the use
of reduced inten
sity conditioning regimen are suggested to improve patient satisfaction.

Background:
Hereditary angioedema

(HAE) is an autosamal dominant disorder due to mutations in C1INH gene. This
disease characterized by subcutaneous and submucos
al edema. 75% of patients have family history of HAE and 25% of
cases arise from spontaneous mutations. This study investigated characteristics and clinical data of patients with
h
ereditary angioedema without family history of this disease.

Materials and M
ethods:

We registered all patients with Angioedema symptoms who had been referred by Physician
from whole of Iran to Immunology, Asthma and Allergy research institute for evaluating the qualitative and quantitative
levels of C1inhibitor from 2004 to 2012.
All patients who were confirmed hereditary angioedema according to clinical
symptoms and laboratory results were visited again to obtain information and published identification card and
educational brochure for hereditary angioedema.

Results:

From 11 patients without family history of Hereditary angioedema(6 mail and 5 female) 55% had type 1 HAE
and 45% had type 2. The mean age of onset of symptoms was 18 years (range: 1.5 to 53 years). laryngeal edema
occurred in 36% of patients and there was

no report of death from hereditary angioedema in patients.

Conclusion
: According to this study both type of hereditary angioedema were identified in patients without family
history of this disease and episodes of edema occur at various body sites such as

larynx.

Background:
Chronic granulomatous disease (CGD) is a rare primary immunodeficiency with X

linked (XL) or
autosomal recessive (AR) inherita
nce, involving defect in one of the subunits of NADPH oxidize. Affected patients are
susceptible to severe, life

threatening bacterial and fungal infections and excessive inflammation. This review
summarizes data from several studies performed in CGD patie
nts referred to Immunology, Asthma and Allergy
Research Institute (IAARI) over the last decade.

Material and Methods:

types, clinical data, and inheritance pattern of 93 CGD patients from 75 families were
reported. In addition to mutation analysis stud
ies, a few cross

sectional studies on different aspects for available group

Background:

Common variable immunodeficiency (CVID) is a group of many primary immunodeficiencies with a
common set of features (including hypo
-
gammaglobulinemia) and different underlying causes. Producing IL
-
17, Th17
cells induce BAFF and APRIL, two important mediat
ors for B cell survival and activation. A sub
-
population of innate
lymphoid cells (ILCs) is capable of producing IL
-
17. Hence, the aim of this study was to measure Th17 cell specific
genes and ILC count in peripheral blood of CVID patients. As IL
-
9 has sho
wn important in some autoimmune
conditions and some of the CVID patients show some aspects of autoimmunity, we also measured the mRNA level of
this cytokine.

Materials & Methods:

Total mRNA extracted from whole blood samples of 10 CVID patients and 10 norm
al controls,
and IL
-
17, RORC2, IL
-
23R and IL
-
9 genes were measured using qRT
-
PCR. Lineage negative, CD127+, CD161+ ILC
counting in blood samples performed with flow cytometry method.

Results:

This study showed a slight decrease in transcript levels of IL
-
17, RORC2 and IL
-
23R in CVID patients, even
though, it was not statistically significant. Accordingly, counting of ILCs showed that these cells are decreased as well.
Interestingly, in CVID blood samples, IL
-
9 mRNA level was significantly more than in cont
rols.

Conclusions:

We showed that Th17 cells and ILCs (another source of IL
-
17) are decreased in CVID patients and this
could be an explanation for their humoral immune response insufficiencies. In addition, elevation in IL
-
9 gene
expression may shed a ne
w light into the way toward understanding the mechanism of autoimmunity in these patients.
