The Crisis in Orthodontic Education: A Minority Opinion

For the “Fathers of Orthodontics,” it was an article of faith that orthodontics is a “science,” or, at the very least, a
science
-
based calling.




The importance of science was our central, sustaining myth.


Unfortunately, a century of
experience ha
s taught us that literally anything and everything work well enough to pay the bills.


In the end, nobody
dies from anchorage loss.


History, it would seem, reveals its alternatives neither to us nor to the patient.
Unfortunately, when everything works,
nothing really matters.


Accordingly, we argue about “evidence based
orthodontics,” apparently because some see it as a potential impediment to the orderly flow of commerce.


Were
orthodontics to be a real science, we would be arguing about the evidence, n
ot the need for evidence.


If we were to
engage in such a dialogue, “teaching” would be an exciting prospect.


To the small pool of smart, slightly eccentric free
spirits who commonly might be expected to enter our academic pipeline, such a dialogue seems
neither to exist nor to
be desired.


Therein lies the rub.


Not every student is burdened by excessive debt.


Not every student has a family.


Not
every student views the university and the things universities do as evils to be endured to get a union card.


Some are
smart enough and sufficiently unencumbered and free
-
spirited enough to contemplate a career of exciting exploration
and discovery.



We don’t need many.


Given the modest needs of our small educational establishment, why then is
there a crisis?


To me, the answer is simple.


The real work product of the orthodontic academic is not just to churn out
more orthodontists.


Rather, if we are to be a real part of the university, we are also charged with generating new
knowledge.


That’s the thing prosp
ective academics find fascinating.


If, however, there is no real market for this work
product, all of our smart young residents will go into practice.


After all, there
is

a market for straight teeth.


In short, we
are in the midst of a crisis of faith.


What, then, is to be done?

I will try to convey a notion of why people go into
“teaching” (it isn’t “bad hands”) and argue that the solution to our problem, lies not so much with organized
orthodontics as it does with the membership as a whole.


As in
“the Field of Dreams,” if the specialty truly values what
academics do, an adequate number will emerge from the student body, more or less like the ghostly ballplayers from
the Iowa cornfield.


Seen in this light, perhaps there is no solution; however, if
there is one, it will, in the end, have to
come from the rank and file.


“If you build it, they will come.”


If you value what an academic does, they will come.


Simple, but definitely not easy.

Lysle E. Johnston, Jr., is professor em
eritus of dentistry at the University of Michigan in Ann Arbor and professor
emeritus of orthodontics at Saint Louis University. He graduated from the University of Michigan School Of Dentistry in
1961 and from Michigan’s Horace H. Rackham School of Gradu
ate Studies (orthodontics) in 1964. He studied anatomy
at the Queen’s University of Belfast (1961
-
2) and received a Ph.D. in anatomy from Case Western Reserve University in
1970. Before becoming chair of orthodontics and pediatric dentistry at the Unive
rsity of Michigan (1991
-
2004), he
chaired the orthodontic departments of Case Western Reserve University (1971
-
6) and Saint Louis University (1976
-
91).
Currently, he is a part
-
time teacher at all three of his former departments.

t successful Olympic champions. He was America’s most highly decorated Olympian
at the 1976 Games in Montreal (the second highest ever, at the time) earning four gold medals in swimming, each in
world record time. Naber became the first swimmer in history

TMJ Disorders: Current & Future Innovations in Diagnostics & Therapeutics

Because their etiologies and pathogene
sis of TMJ diseases are poorly understood, they are difficult to diagnose and
manage. All current approaches to treatments of TMJ diseases are largely palliative. Definitive and rational diagnoses or
treatments can only be achieved through a comprehensive
understanding of the etiologies, predisposing factors, and
pathogenesis of TMJ diseases. While much work remains to be done in this field, novel findings in biomedicine and
developments in imaging and computer technologies are beginning to provide us with
a vision of future innovations in
the diagnostics and therapeutics of TMJ disorders. These advances include the identification and use of local or systemic
biomarkers to diagnose disease or monitor improvements in therapy; the use of imaging technologies f
or earlier and
more sensitive diagnostics; and the use of biomedicine, biomimetics, and imaging to design and manufacture
bioengineered joints. Such advances are likely to help to customize and enhance the quality of care we provide to
patients with TMJ d
isorders.

Dr. Sunil Kapila is the Robert W. Browne Professor and Chair of the Department of Orthodontics and Pediatric Dentistry
at the University of Michigan. Dr. Kapila obtained his dental education at the University of Na
irobi, Kenya. He completed
his orthodontic education at the University of Oklahoma on a Fulbright
-
Hayes Scholarship, following which he pursued
Ph.D. in Oral Biology at the University of California San Francisco. Dr. Kapila has received several research a
wards
including the AADR Hatton Award, the AAO Milo Hellman Award, and the B.F. Dewel Honorary Research Award from the
AAOF. Dr. Kapila is a Diplomate of the American Board of Orthodontics. Dr. Kapila’s NIH

funded research focuses on the

r TMJ degeneration and elucidating the role of specific enzymes in periodontal bone loss. Dr. Kapila

Estradiol Dysfunction in Women with Condylar Resorption

Condylar resorption has been described in the literature for many years.


Many papers have enumerated the possib
le
causes of condylar resorption but the exact mechanism remains elusive.


Because of the female predilection, it has been
assumed for many years that the cause could be related to hormonal influences.


There remains, however, a paucity of
data to prove an
ything for certain, hence the continuing moniker idiopathic condylar resorption. In an attempt to
clinically evaluate the influence of sexual hormones on condylar resorption, we reviewed the hormonal histories and the
sexual endocrine function of a group
of females who presented to our offices with resorption resulting in significant
radiographic condylar changes and skeletal and dental discrepancies.


The data were reviewed to elucidate any possible
correlation between sexual hormone dysfunction and condy
lar resorption.

, it is expected to result in minimal side effects in other part of dentition. Therefore, it is now possible for a
clinician to propose a less compromised treatment plan in mechanically challenging malocclusions. They provide
especially useful anchorage i
n the treatment of more challenging malocclusions that either requires a surgery or
multiple prosthetic restorations if microimplants were not available. This presentation will share the speaker’s
experience of microimplant anchorage in the treatment of c
